Acil Tıp Merkezlerinin Mekansal İlişkiler Açısından İstanbul İli Örneğinde İncelenmesi

Abstract

Ülkemizde Acil Tıp Merkezleri öncelikle eğitim ve araştırma hastaneleri bünyesinde kurulmuştur. Bu merkezlerin kurulması 1990’lı yılların başında acil tıp uzmanlığının bilim dalı olarak kurulmasına denk gelmektedir. Gelişerek değişen acil tıp hizmeti anlayışı sonucunda bu merkezler mimari mekansal organizasyonları açısından, sağlanan acil tıp hizmet akışına cevap veremez hale gelmiştir. Çalışmanın amacı, acil tıp merkezlerinin mimari mekansal organizasyonların sunulan tıp hizmetleri akışına göre düzenlenmesi konusunun araştırılmasıdır. İkinci bölümde, sağlık hizmetleri ve sağlık kurumlarının tanım ve sınıflandırılması yapılmıştır. Hastane sınıflandırmasında Acil Tıp Merkezleri, hizmet türüne göre özel dal hastaneleri kapsamındadır. Üçüncü bölümde ise acil sağlık hizmetleri ve ambulans sistemi ele alınmıştır. Ambulans hizmetleri, vakaya olay yerinde ilk müdahalenin yapılmasıyla başlar ve acil tıp merkezlerine ulaştırılması sırasında bakımıyla devam eder. Bu nedenle acil sağlık hizmetleri organizasyonunda ambulans örgütlenmesi önemlidir. Bölüm, ülkemizde ve İstanbul ilinde acil sağlık hizmetlerinin anlatılmasıyla bitirilmiştir. Dördüncü bölüm Acil Tıp Merkezlerinin ele alındığı bölümdür. Acil Tıp Merkezlerinin tanım ve sınıflandırması yapılmış ve bu merkezlerde sağlık hizmetleri bölümünde bulunan üniteler kapsamlı biçimde anlatılmıştır. Ünitelerin detaylı anlatımdan sonra değerlendirmelerde kullanılacak kontrol listesinde mekansal düzeyde ilişkiler konu edilmiştir. Kontrol listesi, sağlık hizmetleri bölümünde bulunan mekanların anlatımlarından faydalanılarak oluşturulmuş, matris ve şema açıklamalarıyla desteklenmiştir. Beşinci bölümde, oluşturulan kontrol listesine göre ikisi de eğitim ve araştırma hastanesi bünyesinde bulunan “Lütfi Kırdar Kartal Eğitim ve Araştırma Hastanesi Suzan Yazıcı Acil Tıp Merkezi” ve “Haydarpaşa Numune Eğitim ve Araştırma Hastanesi Koç Acil Tıp Merkezi” değerlendirilmiştir. İnceleme sonucunda, mekansal ilişkilerde gözlemlenen eksiklikler ve hatalar sonuç bölümünde sunulmuştur. Çalışma, “acil sağlık hizmetleri” ve “acil tıp merkezleri” konusunda geliştirilecek araştırmalar için ön çalışma niteliği taşımaktadır.

The Emergency Medical Centers in our country have firstly been formed in the structure of educative and research hospitals. The formation of these centers coincides with the formation of emergency medical specialism as a scientific discipline in the early 1990’s. Due to the changing thru developing emergency medical service concept, these centers aren’t able to provide sufficient emergency medical service because of their architectural spatial organizations. The purpose of the study is to research the arrangement according to the influx of the presentation of the emergency medical centers’ architectural spatial organizations. In chapter two, the definition and classification of the health services and institutions are made. In the hospital classification, The Emergency Medical Centers are in the inclusion of special division hospitals according to kids of services rendered. And in chapter three, the emergency health services and the ambulance system is under discussion. The ambulance services commence with the initial intervention on location and continue with the treatment en route to the emergemcy medical centers. Thus, ambulance organization is essential in the organization of emergency health services organization. The part is concluded with the explanation of the emergency health services in our country and the city province of Istanbul. Chapter four is where the Emergency Medical Centers are under discussion. The definition and classification of the Emergency Medical Centers are made and the units existing in the health services department of these centers are explained in detail. The spatial interactions have been the subject in the checklist that is to be utilised after the detailed expression of the units. The checklist has been developed benefiting from the spatial expressions existing in the health services department. This has been supported by the matrix and scheme statements. In chapter five, the “Lütfi Kırdar Kartal Eğitim ve Araştırma Hastanesi Suzan Yazıcı Acil Tıp Merkezi” and the “Haydarpaşa Numune Eğitim Ve Araştırma Hastanesi Koç Acil Tıp Merkezi” which both are, according to the control list developed, under the structure of educative and research hospital; are evaluated. As the result of the investigation, the short comings and the errors in the spatial organization observed are presented in the final part. The study is considered to be an initial study on the subjects of “emergency health services” and “emergency medical centers”.
